Confidentiality Statement
Lived Experience Solutions (LEXs)

At Lived Experience Solutions (LEXs), confidentiality is a foundational part of building trust, safety, and authentic connection across all therapeutic, coaching, supervision, and professional relationships. We recognise that sharing personal experiences, emotions, and vulnerabilities requires courage, and we are committed to holding that information with care, respect, and clear ethical boundaries.

Our commitment to confidentiality

LEXs operates in alignment with the Australian Privacy Act 1988, the AASW Code of Ethics, and relevant professional standards. Information shared with LEXs is treated as confidential and safeguarded accordingly. This includes information provided through:

  • therapy, coaching, or supervision sessions

  • intake forms and written communication

  • phone, video, or email correspondence

Your personal information, session content, and notes will not be disclosed to others without your consent, except where disclosure is required or permitted by law, or where ethical and legal exceptions apply as outlined below.

Limits to confidentiality

In limited circumstances, confidentiality may need to be breached. This may occur where there is:

  • a serious and imminent risk to your life or the life of another person

  • a legal obligation to disclose information (for example, a court subpoena)

Where possible and appropriate, and where it is safe and lawful to do so, LEXs will aim to discuss the need to share information with you before any disclosure occurs.

Record keeping and storage

Client records may include brief session notes, intake information, and relevant correspondence. Records are stored securely using password-protected and encrypted digital systems, or locked physical storage, and are accessed only by the practitioner.

LEXs does not sell or share client data with third parties. Records are retained in accordance with Australian privacy legislation and professional obligations, generally for a minimum of seven years following the last contact, or longer where required by law.

De-identified information may be discussed in professional supervision as part of maintaining safe, ethical, and reflective practice. No identifying details are shared in these discussions.

Data use and administration

For administrative and billing purposes, LEXs collects and stores basic contact details such as name, email address, phone number, and address. This information is used only for purposes related to service delivery, communication, and account management.

Sensitive clinical information is not stored in email or text messages. Clients are encouraged to avoid sharing confidential or sensitive details through unencrypted communication channels.

Your rights

You have the right to:

  • request access to your personal information

  • request correction of factual inaccuracies

  • withdraw consent for information sharing, unless disclosure is required by law
